Monthly Shot to Lower Triglycerides and Pancreatitis Risk

Tryngolza (olezarsen) — ApoC-III ASO for severe hypertriglyceridemia (sHTG) and FCS

Medcelerator Brief

This is for adults with severe hypertriglyceridemia (fasting TG 500 mg/dL or higher) — and caregivers — who need a labelled way to lower triglycerides and cut the risk of acute pancreatitis, and for adults with familial chylomicronemia syndrome (FCS) already on the FCS indication. It explains the 24 June 2026 FDA expansion of Tryngolza (olezarsen) for sHTG (USPI Revised 06/2026; NDA 218614/S-004), monthly subcutaneous autoinjector dosing (50 mg start for sHTG; 80 mg for FCS), CORE and CORE2 trial results, liver-enzyme monitoring, and why this is not a statin swap or a DIY import — without assuming every plan funds it. Authorized is not funded.

Clinician one-pager

Tryngolza is an apolipoprotein C-III (apoC-III)–directed antisense oligonucleotide (olezarsen) given as a once-monthly subcutaneous autoinjector. Initial U.S. Approval: 2024 for FCS; Recent Major Changes June 2026 added the sHTG indication to reduce triglycerides and the risk of acute pancreatitis in adults with fasting TG ≥500 mg/dL (USPI Revised 06/2026; NDA 218614 supplement 004). FDA Notable Approval bulletin 24 June 2026: first treatment shown to reduce acute pancreatitis risk in adults with severe hypertriglyceridemia. Distributed by Ionis Pharmaceuticals, Inc. AE line: 1-833-644-6647 or FDA MedWatch 1-800-FDA-1088. This is not a statin, not a fibrate replacement by itself, and not approved for children.

Distinct path — not this carton: Other TG-lowering medicines (fibrates, omega-3s, niacin, volanesorsen where available, etc.) have different labels and do not carry the same FDA-stated pancreatitis-risk reduction claim in sHTG that Tryngolza’s June 2026 sHTG indication does. FCS and sHTG share this molecule but have different approved doses on the USPI.

Where this is taking place

Commercial labelled door today: United States — USPI indications as adjunct to diet: (1) reduce TG in adults with FCS; (2) reduce TG and risk of acute pancreatitis in adults with sHTG (TG ≥500 mg/dL). Autoinjectors: 50 mg/0.8 mL (NDC 71860-102-01) and 80 mg/0.8 mL (NDC 71860-101-01). Store refrigerated 2–8 °C; room-temperature window up to 6 weeks in original carton per USPI.

Outside the United States: Health Canada / EMA/EC / MHRA / TGA / PMDA / Swissmedic / NMPA authorisation for the sHTG pancreatitis indication not confirmed on sources used for this draft. Do not treat a US autoinjector as another country’s carton. Do not import on your own.

Evidence trials (completed — not enrolment doors for newly labelled commercial use):

  • Trial 2 / CORE — NCT05079919 (sHTG)
  • Trial 3 / CORE2 — NCT05552326 (sHTG)
  • Trial 1 — NCT04568434 (FCS)

Sponsor framing on USPI: Ionis. Combined sHTG safety population 1,061 adults (705 Tryngolza; 356 placebo).

Access by country

  • United States: Ask a lipidology / endocrinology / cardiology / gastroenterology clinician managing very high triglycerides about labelled Tryngolza if fasting TG is ≥500 mg/dL (sHTG) or if you have genetically identified FCS. Path: confirm indication → low-fat diet counselling → liver enzymes before start / dose increase → train on autoinjector → monthly SC injection (abdomen or front of thigh; upper-arm only if HCP/caregiver injects). Support: www.TRYNGOLZA.com or 1-833-644-6647. No prices in this article.
  • Other countries: Authorisation not confirmed here for the sHTG door — ask what legal local options exist; do not import on your own.

Who is eligible

United States (USPI 06/2026):

  • FCS: Adjunct to diet to reduce triglycerides in adults with familial chylomicronemia syndrome. Dose: 80 mg SC once monthly. Instruct FCS patients to consume ≤20 g fat/day.
  • sHTG: Adjunct to diet to reduce TG and the risk of acute pancreatitis in adults with severe hypertriglyceridemia (TG ≥500 mg/dL). Dose: start 50 mg SC once monthly; if tolerated and additional TG reduction clinically indicated, may increase to 80 mg SC once monthly. Assess TG when clinically appropriate; TG-lowering effect may be measured within 3 months.
  • Contraindication: History of serious hypersensitivity to olezarsen or any excipient.
  • Pediatrics: Safety and effectiveness not established.
  • Pregnancy / lactation: No adequate human data; disease-associated pancreatitis risk in pregnancy discussed on label — clinician conversation required.
  • Renal / hepatic: No adjustment for mild–moderate impairment studied; not studied in severe renal impairment / ESRD or severe hepatic impairment.

Not labelled on sources confirmed for this draft:

  • Children / adolescents.
  • Using Tryngolza as a stand-alone cure for atherosclerotic risk without addressing the rest of the lipid plan.
  • Non-US marketing authorisations asserted here.

What the pivotal study showed (CORE / CORE2 / FCS Trial 1)

sHTG — Trial 2 (NCT05079919) and Trial 3 (NCT05552326): Adults with fasting TG ≥500 mg/dL on optimized stable background lipid-lowering therapy; randomized to Tryngolza 50 mg, 80 mg, or placebo SC every 4 weeks for 53 weeks. Mean baseline TG 1,116 mg/dL (pooled).

  • Primary (Month 6 percent change in fasting TG vs placebo):
    • Trial 2: −63% (50 mg) and −72% (80 mg) placebo-corrected (both p < 0.0001).
    • Trial 3: −49% (50 mg) and −55% (80 mg) placebo-corrected (both p < 0.0001).
  • Adjudicated acute pancreatitis (integrated Trials 2+3, Week 1–53): Pooled Tryngolza vs placebo rate ratio 0.15 (95% CI 0.05–0.40) — 85% reduction in event rate on the USPI integrated analysis; 50 mg RR 0.09; 80 mg RR 0.24. Effect concentrated in the high-risk subgroup (baseline TG ≥880 mg/dL and prior pancreatitis history).

FCS — Trial 1 (NCT04568434): Genetically identified FCS; Tryngolza 80 mg vs placebo. Placebo-corrected TG change at Month 6: −43% (95% CI −74%, −11%; p = 0.0084). Numerical pancreatitis incidence lower on Tryngolza 80 mg (1/22) vs placebo (7/23) over 12 months — supportive narrative on USPI, not the sHTG pancreatitis primary analysis.

How it is taken

SitesAbdomen or front of thigh; back of upper arm only if HCP/caregiver injects
PrepRemove from fridge; warm 30 minutes at room temperature; do not use other heat sources
Missed doseTake as soon as possible; resume monthly from date of most recent dose
DietMaintain low-fat diet; FCS ≤20 g fat/day

Safety (what the label puts first)

  • Hypersensitivity reactions (bronchospasm, diffuse erythema, facial swelling, urticaria, chills, myalgias) — stop and seek care; contraindicated after serious hypersensitivity.
  • Liver enzyme abnormalities / hepatic fat — more frequent at 80 mg than 50 mg in sHTG (≥3× ULN: 7% at 80 mg vs 3% at 50 mg and placebo). Consider LFTs before initiation or dose increase and when indicated; interrupt / reduce for persistent elevations; discontinue for serious hepatic injury with symptoms and/or jaundice.
  • Common AEs: FCS — injection-site reactions, decreased platelets, arthralgia. sHTG — injection-site reactions, transaminase increases.
  • Label also notes platelet decreases, glucose/HbA1c increases, and LDL-C increases with non-HDL-C decreases — discuss with the clinic.

How to talk to a doctor

Bring recent fasting TG results, pancreatitis history, current lipid medicines, liver-test history, diabetes status, and pregnancy plans. Ask: “Do I meet the USPI sHTG (TG ≥500) or FCS population, and should we start at 50 mg (sHTG) or 80 mg (FCS)?” Ask how liver monitoring and autoinjector training will work. Do not stop background lipid therapy or diet measures unless the clinic says so.

Research and regulatory team

  • Distributor / USPI holder: Ionis Pharmaceuticals, Inc., Carlsbad, CA.
  • FDA designations for sHTG indication (FDA bulletin): Priority Review, Breakthrough Therapy.
  • Pivotal NCT IDs: NCT05079919, NCT05552326 (sHTG); NCT04568434 (FCS).
  • Named individual principal investigators are not listed on the USPI highlights used here. Check ClinicalTrials.gov study contacts or ask the prescribing clinic for the research team on CORE / CORE2 / FCS Trial 1.

Bottom line

Tryngolza is a US-labelled monthly apoC-III ASO shot for adults with FCS (TG lowering) and, since 24 June 2026, for adults with sHTG (TG ≥500 mg/dL) to lower TG and reduce acute pancreatitis risk — the first FDA-stated pancreatitis-risk reduction claim in that sHTG setting. The near-term path is a lipid-capable clinician, diet, liver monitoring, and the autoinjector — not a DIY import, and not a guarantee of funding.
